What Is Wegovy?

Wegovy (semaglutide) is a once-weekly injectable weight management medication approved for use in adults with obesity or excess weight alongside at least one weight-related health condition. It works by mimicking a hormone called GLP-1, which helps regulate appetite, slow digestion and improve blood sugar control. The result, for most people, is a reduced desire to eat and a greater sense of fullness after meals.

The Wegovy Dosing Timeline: Stage by Stage

Wegovy follows a carefully structured escalation schedule. The gradual increase is intentional. It reduces the risk of gastrointestinal side effects and gives your body time to adjust to each new level before moving higher.

Stage 1: Weeks 1 to 4 — 0.25 mg

This is your introductory dose and is not intended to produce significant weight loss. Its sole purpose is to help your body get used to the medication. Most people tolerate this stage well, though mild nausea, constipation or fatigue can occur.

Tips for this stage:

  • Eat smaller, more frequent meals
  • Stay well hydrated throughout the day
  • Avoid high-fat or very rich foods that may worsen nausea
  • Inject on the same day each week to build a routine

Stage 2: Weeks 5 to 8 — 0.5 mg

Your first dose increase. At this point, appetite suppression begins to become more noticeable for many patients. Side effects from stage one often start to ease, though some people experience a brief return of nausea when moving to a new dose.

Stage 3: Weeks 9 to 12 — 1 mg

By this stage, most people are seeing tangible changes in appetite and beginning to see weight loss results. Energy levels can begin to stabilise and food noise, the constant background thinking about food, typically starts to reduce.

Stage 4: Weeks 13 to 17 — 1.7 mg

This is the penultimate escalation stage. Weight loss progress tends to continue steadily here. Your prescriber will review your response and tolerance before advising whether to move to the maintenance dose.

Stage 5: Week 18 onwards — 2.4 mg (maintenance dose)

The 2.4 mg dose is the full therapeutic maintenance dose and is where most patients remain long term. According to the NHS, Wegovy at this dose has been shown to help people lose an average of around 15% of their body weight when combined with a reduced-calorie diet and increased physical activity.

Some patients may remain on 1.7 mg if they are responding well or experiencing side effects at the higher dose. Treatment is always tailored to the individual.

What Side Effects Should You Expect?

Side effects are most common when starting treatment or moving to a higher dose. They typically settle within a week or two. The most frequently reported include:

  • Nausea, particularly after eating
  • Constipation or diarrhoea
  • Headaches or dizziness
  • Fatigue, especially in the first few weeks
  • Injection site reactions such as redness or mild irritation

More serious side effects are uncommon but should be reported to a healthcare professional straight away. These include persistent severe stomach pain, signs of an allergic reaction or any signs of pancreatitis, such as persistent severe abdominal pain radiating to the back.

Frequently Asked Questions About Wegovy Dosing

Can I skip a dose escalation stage if I am tolerating Wegovy well?
No. The titration schedule exists to protect your safety and improve long-term tolerability. Even if you feel you are coping well, moving through the stages too quickly increases the risk of gastrointestinal side effects. Always follow your prescriber’s guidance.
What happens if I cannot tolerate the dose increase?
Your prescriber may advise you to remain on your current dose for an additional four weeks before trying again. In some cases, the dose may be reduced temporarily. This is entirely normal and does not mean treatment has failed.
How long will it take to see results with Wegovy?
Some patients notice appetite changes within the first few weeks, but meaningful weight loss typically becomes more apparent from stage three onwards. Most clinical results are observed over a period of several months at the maintenance dose. Making lifestyle changes will support progress, and our team is on hand to help you maintain motivation and progress.
Do I have to stay on Wegovy indefinitely?
Wegovy is generally considered a long-term treatment. If you stop taking it, appetite and weight can return to previous levels over time. Your prescriber will discuss ongoing treatment plans with you as part of regular reviews.
Can I switch from Wegovy to Mounjaro?
It is possible to switch between weight loss medications, but this should always be done under the guidance of a prescriber. At Nettles Pharmacy in London, our team can review your current treatment and discuss whether switching would be appropriate for your situation.
Is Wegovy available on the NHS?
Wegovy is available through specialist NHS weight management services in England, though access criteria apply. It is also available privately. Contact us to discuss your options with our team in London.
