Starting a new injectable medication can feel daunting, particularly if you have never self-injected before. One of the most common questions we hear from patients beginning their weight management journey with Mounjaro is simply: am I doing this correctly? Getting the injection site and technique right from the very beginning makes a genuine difference, both to your comfort and to how well the medication works.
Our team at Nettles Pharmacy in Tooting is here to make sure you feel confident at every stage of your treatment. If you have questions about your Mounjaro injections or would like hands-on guidance, contact us or book now to speak with our pharmacy team.
Where Can You Inject Mounjaro?
Mounjaro (tirzepatide) is administered as a subcutaneous injection, meaning it is injected into the fatty tissue just beneath the skin. There are three recommended injection sites:
- Abdomen – the area around your stomach, at least 5 cm away from the navel
- Upper thigh – the front or outer area of either thigh
- Upper arm – the back or outer area of the upper arm (usually easier with assistance)
Each site is equally effective for delivering the medication. The most important rule is to rotate injection sites regularly. Using the same spot repeatedly can lead to a build-up of scar tissue beneath the skin, which can affect how well the medication is absorbed.

Injection Site and Dosage Reference Table
| Treatment Stage | Dose | Duration | Recommended Injection Sites | Rotation Tip |
|---|---|---|---|---|
| Introduction | 2.5 mg | Weeks 1–4 (4 injections) | Abdomen, thigh or upper arm | Begin a rotation pattern from the start |
| Early Treatment | 5 mg | Weeks 5–8 (4 injections) | Abdomen, thigh or upper arm | Avoid any areas still tender from previous injections |
| Intermediate | 7.5 mg | Weeks 9–12 (4 injections) | Abdomen, thigh or upper arm | Alternate between body areas each week |
| Higher Dose | 10 mg | Weeks 13–16 (4 injections) | Abdomen, thigh or upper arm | Keep a simple rotation log if helpful |
| Advanced | 12.5 mg | Weeks 17–20 (4 injections) | Abdomen, thigh or upper arm | Check skin between injections for any changes |
| Maximum | 15 mg | Week 21+ (ongoing weekly) | Abdomen, thigh or upper arm | Consistent rotation remains important at all stages |
Not every patient will progress through all dose stages. Your prescriber will assess your response and tolerance before recommending any increase.
Step-by-Step Injection Guide
Follow this step-by-step process each time you administer your Mounjaro injection. Taking a calm, methodical approach will help you feel more comfortable, especially in the early weeks of treatment.
- Wash your hands thoroughly with soap and warm water for at least 20 seconds before handling the pen. Dry with a clean towel.
- Check the pen before use. Confirm it is the correct dose, check the expiry date and inspect the liquid through the window. It should be clear and colourless to slightly yellow. Do not use if it appears cloudy or contains particles.
- Allow the pen to reach room temperature if it has been stored in the fridge. Leave it at room temperature for around 30 minutes before injecting. Cold medication can cause unnecessary discomfort.
- Choose your injection site and clean the area with an alcohol wipe. Allow the skin to dry fully before proceeding.
- Remove the base cap from the pen. Do not remove the clear cap yet. Check that the yellow-green needle cap is visible and intact.
- Remove the clear cap just before injecting. Do not put it back on once removed.
- Place the pen flat against the skin at the chosen site. Do not angle it. The pen should sit flush against your skin.
- Press and hold the orange injection button firmly until you hear a click. Keep the pen pressed against the skin and continue holding for around 10 seconds to ensure the full dose is delivered.
- Lift the pen away from the skin. The needle will retract automatically. Check the window on the pen to confirm the yellow-green bar is fully visible, which indicates the complete dose has been administered.
- Dispose of the used pen safely in an approved sharps bin. Do not recap the needle. Never dispose of used pens in household waste.
If you are ever unsure whether the full dose was delivered, do not administer another injection. Contact our team for guidance before your next scheduled dose.
Tips for a More Comfortable Injection
Small adjustments to your technique can significantly reduce discomfort over time. Consider the following:
- Always allow the pen to warm up before injecting
- Ensure the skin at the injection site is fully dry after cleaning with an alcohol wipe
- Relax the muscle in the area you are injecting into
- Inject slowly and steadily, keeping the pen flat against the skin
- Rotate sites consistently and keep a simple note of where you last injected
- Avoid injecting into areas that are bruised, broken, scarred or irritated
- Do not rub the skin after injection, as this can cause irritation
How to Store Your Mounjaro Pen
Correct storage is just as important as correct technique. Mounjaro pens should be:
- Stored in the refrigerator between 2°C and 8°C
- Kept away from the freezer compartment and not stored next to ice packs
- Protected from direct light
- Used within 21 days if stored at room temperature (below 30°C) after removal from the fridge
- Kept out of reach of children at all times

FAQs: Mounjaro Injection Sites and Technique
Click on each FAQ to reveal the answers you are looking for:
Which injection site is best for Mounjaro?
All three recommended sites (abdomen, thigh and upper arm) are equally effective. The abdomen is often the most convenient for self-injection. The most important thing is to rotate between sites regularly to prevent tissue build-up.
How far from the navel should I inject?
When injecting into the abdomen, always stay at least 5 cm (approximately two finger widths) away from the navel. Avoid the area directly around your belly button, as the skin can be less suitable for subcutaneous injections.
Can I inject Mounjaro in the same spot every week?
No. Repeated injections in the same spot can lead to lipohypertrophy, a build-up of fatty tissue beneath the skin that can affect medication absorption. Rotating sites each week is strongly recommended.
What should I do if I miss a dose?
If you miss a dose and it has been fewer than four days since your scheduled injection day, administer the missed dose as soon as you remember. If more than four days have passed, skip that dose and resume your normal weekly schedule. Never administer two doses in the same week.
What if I see blood after injecting?
A small spot of blood at the injection site is normal and nothing to worry about. Apply gentle pressure with a clean cotton pad. Do not rub the area. This is usually caused by a very small blood vessel being nicked and does not indicate a problem with the injection.
Can I inject into my upper arm myself?
It is possible but can be more difficult to manage alone. Many patients find it easier to use the abdomen or thigh for self-injection. If you prefer the upper arm, it may be easier with the help of another person. Ask our team if you need practical guidance.
My pen did not click. Did I receive the full dose?
The click confirms the injection has started. After the click, it is important to continue holding the pen against the skin for 10 seconds. If you are unsure whether the dose was fully delivered, check the window for the yellow-green indicator bar and contact our pharmacy team before taking any further action.
Can I inject through clothing?
It is not recommended. Always expose the skin directly and clean the area before injecting to minimise infection risk and ensure the injection is administered correctly.
